The E-Forms Tab allows you to choose which Enrollment E-Forms will display in the Student Records Portal (SRP) for the selected Site. The E-Forms the Primary Guardian are required to fill out will display in the SRP/Documents Tab when they are enrolling their Student(s). Whenever the Primary Guardian opens the E-Forms, they can enter the information directly into each field just as they would write the information on a paper form.
NOTE: We have pre-designed the Guardian Attestation, Home Language Survey, Homeless Enrollment Form, Economically Disadvantaged Form, Title VI ED 506 Indian Student Eligibility Form, Lunch Room Application, Medicaid/SoonerCare Disclosure, OSSAA Eligibility Record Form, Title 1/Part C Family Survey Form and the Parental Consent Form. However, you can edit those forms using Report Designer, if necessary.
We also have a pre-designed Site Policy Form template which allows you to collect the values for the following indicators in the By Year Info Tab: AUP Agreement, Internet Permission, School Policy Agreement, Permission to Publish Picture, and Permission to Publish Work. When the E-Form is posted, the values in the By Year Info Tab for the Student will be updated accordingly.
IMPORTANT NOTE: The editor controls for each of the fields has a unique ID that is used to identify the values. Do NOT change this ID. It is used to update the By Year Info with the correct value.
