Student Records Portal: Updated the Title VI ED 506 Indian Student Eligibility Form. This updated form will now collect the values for the following indicators in the Demographics Person Info.
Tribal Affiliation (Tribe or Band Name)
Roll Number (Enrollment Number)
Side Family (Child, Parent, Grandparent)
When the E-Form is posted, the values in the Demographics Person Info for the Student will be updated accordingly.
In addition, the following rules have been applied to the form when it is being completed by the parent/guardian.
The name and address of the Tribe or Band is required. The name is a drop-down list of the Tribal Affiliation codes as defined by the Wave
The individual with tribal membership is required. If the child’s parent or child’s grandparent is checked, the name of the individual with tribal membership is required.
The Tribe or Band recognition selection is required.
Proof of membership in Tribe or Band is required.
If provided, the enrollment number cannot be more than 10 characters.
IMPORTANT NOTE: The editor controls for each of the fields has a unique ID that is used to identify the values. Do NOT change this ID. It is used to update the Demographics Person Info with the correct value.
IMPORTANT NOTE: Title VI ED 506 Indian Student Eligibility Forms submitted by the parent/guardian prior to 04/15/2022 do not update the Demographics Person Info. For those submitted E-Forms, you must update Demographics Person Info manually just as you did before this update.
IMPORTANT NOTE: If you are using a user-designed version of the Title VI ED 506 Indian Student Eligibility Form, it will not update the Demographics Person Info. For these updates to be applied to the E-Form, you must re-design this E-Form. If you are using the default template of the Title VI ED 506 Indian Student Eligibility Form, you do not need to re-design this E-Form.
· Student Records Portal: Added a new Site Policy Form template. This new form will collect the values for the following indicators in the By Year Info.

School Policy Agreement
Permission to Publish Picture
Permission to Publish Work
When the E-Form is posted, the values in the By Year Info for the Student will be updated accordingly.
IMPORTANT NOTE: The editor controls for each of the fields has a unique ID that is used to identify the values. Do NOT change this ID. It is used to update the By Year Info with the correct value. You can make other changes to the E-Form, such as removing the indicators you do not want to collect or changing the layout.
IMPORTANT NOTE: If you manually designed an E-Form to collect these indicators, it will not update the By Year Info. If you want to take advantage of the features of this new E-Form, you must use the default version or a newly designed version of this E-Form.
· Options | Site Tools | Student Records Submissions: Added a new option Remind Selected to send a reminder to the parent/guardian for ‘Guardian In Process’ submissions.
· Options | Site Tools | Student Records Submissions: Added the time stamp for Date Submitted and Date Posted.
· Options | Site Tools | Student Records Submissions: When a submission is rejected, you now have the ability to specify a comment to include in the email.
The Comments of the submission will also be updated with the specified comment.
· Options | Site Advanced Setup | Student Records Portal | E-Forms: Added the ability to attach Document Uploads to an E-Form.
This will allow you to specify attachments for E-Forms when necessary. The required status of the Document Upload can also be specified.
· Options | Site Setup | Settings | Grade Settings: Added a new date range to specify when the Course Request Entry is available for the Grade level.
The Enable Course Request Entry option has been removed from the LEA Settings.
IMPORTANT NOTE: If you currently have the Course Request Entry enabled in the LEA Settings, you must specify the Course Request Entry Begin Date and Course Request Entry End Date to re-enable the option.
